Medical Records
A patient, or his/her legal representative, may inspect and/or obtain a
copy of their medical records, or have copies of medical records sent
to another facility. Kaweah Delta Health Care District requires a completed
and signed authorization and valid identification before releasing any
documents to anyone, including the patient.
Request a copy of your medical records:
Patients can request copies of their medical records electronically by
using the link
https://www.hxe.io/hipaa_form/kdhcd (What is hXe.io?) or by completing our form entitled “Authorization for Use or Disclosure
of Health Information” and by presenting valid identification. The
completed authorization must be dated and signed. You can download and
print the authorization form (below) or we can fax or email you an authorization.

The completed authorization may be mailed, faxed, or emailed.
Medical Provider Requests for Medical Records/Results
For continuity of care purposes, please fax or email your request on your
letter head and include the following information on your request:
- Patient Name
- Date of Birth
- Dates of service being requested
- Type of reports being requested

Patient Requests for Medical Records/Results
Patients can have medical records be sent by:
Patients can request their medical records be provided to them in the following formats:
Processing times may vary depending on the complexity of your request,
however in most cases authorizations are completed within two to three
business days from the date of receipt.
Valid identification:
Medical records will not be released without the requestor providing proper
identification.
A government issued identification is required. If you are requesting records for someone other than yourself, you must
provide supporting legal documentation such as:
- Power of Attorney
- Advance Health Care Directive
- Living Will
- Executor of the Estate
- Court order/Conservatorship appointed by the court

Patient Portal:
How to access your health information
You can access your health information on our new upgraded online patient
portal “MyHealth Kaweah Delta”. This new portal will allow
you to access your test results as soon as they are available.
All invitations for our new patient portal come via e-mail. If you would
like to receive an invite prior to your next visit, call us at 559-624-5120 or email
kdmyhealth@kdhcd.org.
Once you create an account, it will contain health information from future
Kaweah Delta visits.
How to obtain birth and death certificates:
Kaweah Delta Medical Center does not retain copies of birth or death certificates.
They are maintained by the Tulare County Clerk.
