James McNulty, PharmD is the Director of Pharmacy Services for Kaweah
Delta Health Care District. He has served in this capacity for over 10
years. He and his team have been nationally recognized multiple times
for pharmacy achievements, including ASHP Best Practice Awards in 2010

Rheta Silvas, PharmD, Assistant Director of Pharmacy
Rheta Sandoval, PharmD is the Assistant Director of Pharmacy for Kaweah
Delta Health Care District. She has been a Clinical Pharmacist for over
20 years, working at KDHCD for over 15 years, serving first as the Inpatient
Pharmacy Manager for over 10 years before moving into her current role
as Assistant Director. She currently serves as a preceptor for PGY1 pharmacy
residents in the area of practice management. In collaboration with others
on the Pharmacy Management Team and staff, Dr. Sandoval was involved in
the strategic planning for and implementation Medication-related technologies
including Order Management Systems, Smart Pumps, Automated Dispensing
Cabinets (ADC), Anesthesia ADC's, Automated Parenteral Nutrition Compounder,
IV Workflow Technology, Bar Code Medication Administration, Automated
Inventory Management and Packaging Equipment. In collaboration with Department
Leadership and staff, she assisted in the establishment of Clinical Pharmacy
Services including Anemia Management, Antimicrobial Stewardship, Inpatient
Anticoagulation, Emergency Medicine, Pain Management, Medication Safety,

Nicole Gann, PharmD, BCPS, Clinical Coordinator/PGY1 Residency Program Director
Nicole Gann, PharmD is the Clinical Coordinator and PGY1 Residency Program
Director for Kaweah Delta Health Care District and is a Clinical Assistant
Professor (WOS) for the University of California, San Francisco School
of Pharmacy. She has worked at KDHCD for over 15 years and currently serves
as a preceptor for PGY1 pharmacy residents in the area of practice management
and as the site coordinator for pharmacy student rotations including students

Amanda Bringe, PharmD
Amanda Bringe, PharmD is the Inpatient Coordinator at Kaweah Delta Health

Medication Safety:
Sarah Stephens, PharmD, BCPS, CPPS
Sarah Stephens, PharmD, BCPS is the Medication Safety Coordinator for
Kaweah Delta Health Care District. She serves as a preceptor for PGY1
pharmacy residents and pharmacy students from the University of California,
San Francisco. Dr. Stephens’ pharmacy career has spanned over a
decade with a primary clinical focus in acute care adult internal medicine
and 7 years in academia at the University of Utah College of Pharmacy.
Current initiatives and research interests include reducing medication
errors in the areas of anticoagulation and pain management, improving
transitions of care with an emphasis on discharge medication reconciliation,
and evaluating safety culture. Dr. Stephens currently serves as a member
of the California Hospital Association Medication Safety Committee and
the American Society of Health-System Pharmacists Section of Inpatient

David Crowther, PharmD, BCPS, Formulary Management Specialist
David Crowther, PharmD, BCPS is the Formulary Management Specialist for
Kaweah Delta Health Care District and serves as a preceptor for both PGY1
pharmacy residents as well as pharmacy students. He received his Bachelors
of Science in Physiology/Developmental Biology (2006) from Brigham Young
University and his Doctor of Pharmacy (2009) from Roseman University.
Dr. Crowther completed both his PGY1 Pharmacy Residency (2010) and PGY2
Drug Information Residency (2011) at the University of Virginia Health
System in Charlottesville VA. After residency he worked as a Clinical
Specialist in Drug Information at MD Anderson Cancer Center in Houston

Ben Ereshefsky, PharmD, BCIDP
Ben Ereshefsky, PharmD, BCPS is an infectious diseases specialist who
serves as the antimicrobial stewardship pharmacist at Kaweah Delta Health
Care District. He serves as a preceptor for PGY1 pharmacy practice residents
who take an ID/antimicrobial stewardship rotation. Dr. Ereshefsky received
his Doctor of Pharmacy from the University of Texas College of Pharmacy
in 2010. He completed both his PGY1 residency and PGY2 Infectious Diseases
Pharmacy residency at UK HealthCare in Lexington, KY in 2012. Dr. Ereshefsky
comes to KDHCD after spending 4 years in academia at the Philadelphia
