Outpatient Pharmacy Services
Kaweah Delta Health Care District (KDHCD) is an integrated healthcare delivery
system anchored around a 581-bed community-based teaching hospital. With
a complement of ancillary services and ambulatory clinics, KDHCD provides
a broad array of primary and specialty care to a largely rural and medically
underserved community. The Outpatient Pharmacy Department provides services
to the community via a retail pharmacy open to the public, a pharmacy
that serves only Kaweah Delta employees, a home infusion pharmacy, as
well as both pain management and ambulatory care pharmacists working in
the District’s outpatient clinics.
Ambulatory Care and Pain Pharmacy Services
Pharmacists at Kaweah Delta work throughout the District, and have been
operating in the outpatient setting since 2015. Pharmacists are now present
in all the rural health clinics, the Sequoia Health and Wellness Center,
and the Chronic Disease Management Center. In the outpatient clinic settings
pharmacists have patient appointments, manage complex medication regimens,
send prescriptions, and order labs to assess medication effectiveness
and safety. Additionally, pharmacist look at patients on a population
health level by identifying gaps in care and assisting primary care physicians
and specialists with closing those gaps. A pharmacy team also leads the
Employee Wellness Specialty Program that manages medications for patients
with diabetes, chronic infectious disease, and certain rheumatologic conditions.
In the same clinics, pain management pharmacist see patients and help manage
chronic pain. Similarly to the ambulatory care pharmacists, they hold
patient visits, send prescriptions, and order labs. They accept referrals
for patients with chronic pain with the goal of reducing opioid burden,
utilizing non-opioid therapeutics, and improving quality of life.
Kaweah Delta Home Infusion Pharmacy
The Home Infusion Pharmacy is committed to being your world-class home
infusion pharmacy. Our experienced team with over 25+ years in the home
infusion business specializes in intravenous medication and enteral nutritional
needs. We are able to provide all supplies and IV pumps (including portable
options) necessary to take care of your therapy needs. Receive these treatments
in the convenience of your own home as opposed to prolonged hospitalization,
SNF, long-term care placement, or daily trips to an infusion center. Our
team works closely with the Kaweah Delta Hospice and Kaweah Delta Home
Health Services. We are Joint Commission accredited signifying our commitment
to superior patient care. We take all insurance types and have dedicated
intake staff who work directly with your provider and insurance to ensure
authorization for home care services.

- Intravenous Hydration and Electrolyte Replacement

- Enteral Therapy
- Pediatric Infusions and Specialized Infant Formulas
- Injectable Anti-Emetics (chemotherapy, hyper emesis in pregnancy, retractable
nausea and vomiting)
- Intravenous Catheter Care
- Medications and Clinical Support for CAPD and for Hospice of Tulare County

Kaweah Delta Employee Pharmacy
The employee pharmacy is an outpatient pharmacy for Kaweah Delta employees
and family members only. The pharmacy is located on the downtown campus.

Kaweah Delta Pharmacy
The retail pharmacy is open to the public. It is conveniently located downtown
near the hospital’s Emergency Room entrance. Pharmacists in the
retail pharmacy are optimizing adherence to medications at discharge with
our Meds-to-Beds concierge medication delivery program. They also dispense
medications for the Employee Wellness Specialty Program and Medication
Assistance Program, which assists qualifying employees and patients with
lower prescription costs.
