Temporary Change to Visitor Policy for Transitional Care and Subacute Units
Visitation to patients in Subacute units and Transitional Care located
at our South Campus (1633 S. Court Street) is allowed for 30 minute visits
that must be scheduled in advance. Up to 2 visitors from the same household
may visit per patient. Visits will take place either outdoors or in a
large communal space such as a dining room or activity room, not in the
patient room. All visitors must wear a mask and will be screened for symptoms
upon entrance to the building. A staff member will supervise each visit
to ensure proper mask use and social distancing. Visitation days are dependent
upon the staff availability and may vary at each campus.

Kaweah Delta Nursing Center
Skilled Nursing in Tulare County
The skilled nursing center at Kaweah Delta Health Care District provides
a pleasant, relaxed and home-like atmosphere while patients are receiving
a higher level of care than can be given at home. This unit, which is
comprised of beds in our Transitional Care and Subacute Care Units, offers
large, semi-private rooms, a spacious and colorfully furnished dayroom
for visiting, TV viewing or planned activities. A tree-shaded courtyard
provides an area where visitors can be received for more private conversations.
Each person is cared for by an interdisciplinary team which may include:

- A Social Worker
- A Pharmacist
- Physical and Speech Therapists
- Activity Coordinators

Transitional Care
Kaweah Delta's
Transitional Care Services is a 24-bed unit that supports short stay patients with specific therapy
needs. These patients could be individuals in need of extensive wound
care who use devices to facilitate wound healing, patients in need of
long-term antibiotic intravenous therapy, patients in end stage renal
disease that require dialysis, diabetic support, cardiovascular/stroke
support, and cardiac/post open-heart support.
Transitional Care Services emphasizes:
- Physical therapy
- Occupational therapy
- Restorative nursing
- Activity/recreational therapy
- Speech therapy
- End-Of-Life Care
TCS provides specialized care of client for healing wounds, long-term antibiotic
therapy, physical therapy or occupational therapy needs. Our nurses are
highly trained to provide skilled care at end-of-life; hospice services
may also be provided.
Subacute Care
For patients in need of long-term care, our
Subacute Unit houses 30-beds long-term and provides attentive care in need of round-the-clock
assistance.
