Welcome to the Kaweah Delta’s sub-acute program at Community Health Center. We offer technically advanced therapies for the severely neurologically impaired, patients suffering from respiratory failure requiring mechanical ventilation, gastric tube feedings, the terminally ill, and technologically dependent patients. Our staff work as a team, looking at the total patient, working to maximize the potential for rehabilitation. The success of such an approach is evident in the fact that many of our sub-acute patients progress to higher levels of rehabilitation. Residents realize a more comfortable and rewarding life. With the help of highly trained, long-term health care teams, they achieve optimum progress in their recovery.

Reaching Your Potential
Most people have the potential to return home after hospitalization, if given the opportunity to increase their level of independence through rehabilitation therapies. The sub-acute team at Community Health Center gives patients the opportunity to regain their confidence and self-sufficiency in a hopeful, loving environment. We remain sensitive to the concerns and needs of each patient and their family and realize that the art of healing requires compassion, dedication, empathy, and honesty.

A Caring Staff
Our staff on the sub-acute unit consists of a licensed physician, a pulmonologist who sees patients on a consulting basis, registered nurses, CNAs, registered dieticians, licensed pharmacists and respiratory therapists. Additionally, an enterostomal nurse makes regular rounds and is available on a consulting basis. The fundamental objective of the sub-acute unit is to provide an optimum life for the patient and peace of mind for the family. The focus is on providing the best and most appropriate care for each individual patient starting with a customized treatment plan. This highly specific plan is constantly revised to reflect the progress and needs of the patient. Families find peace of mind in knowing that their family members are receiving the best care available.

All patients in need of rehabilitative services, regardless of payer source, receive therapy provided by licensed physical therapists, occupational therapists, and speech therapists. We strive to help patients regain confidence and self-sufficiency by working closely with them to address their specific goals.

Our Larger Facility
The sub-acute unit at Community Health Center is a branch of Kaweah Delta Health Care District, one of the Valley’s premier health care providers who have served Visalia and its surrounding community for 40 years. Its many specialty departments are easily accessible to those who are patients at the sub-acute unit. The main hospital campus is located less than a mile from Community Health Center.

Comforts of Home
Attractive and tasty meals are planned by a licensed dietician and prepared for each person in accordance with doctor’s orders and/or special diet needs. Efforts to make the eating experience an important part of the social activity of the day are encouraged.

Certified Activity Directors involve the community and family in planned events. Music, crafts, and sewing are among activities offered. Recreational activities are important to recovery and social well being. Ministers of various faiths visit and hold religious services in the dayroom on a regular basis. Patient’s personal priest, minister or rabbi is encouraged to visit often.
