Peritoneal Dialysis

The focus of this is to provide optimum management of end stage renal disease services to patients who also require Comprehensive Medical Rehabilitation. This service represents the first opportunity in the State of California for peritoneal dialysis patients to be placed in a rehabilitation center. Patients must meet CRC admission criteria. Pain Rehabilitation Program

Purpose of the program is to eliminate the adverse impact of chronic, unrelieved pain on the life of patients and their families through a comprehensive multidisciplinary pain management program. The goal is to assist the patient in achieving the highest day-to-day function with the lowest possible level of pain. For patients who have not responded to more traditional approaches to pain relief. Please call (559) 624-3906.

Inpatient Pulmonary Rehabilitation

In individually tailored, multidisciplinary program that is formulated through accurate diagnosis, therapy, emotional support, and education that stabilizes or reverses both the physio and psychopathology of pulmonary diseases and attempts to return the patient to the highest possible functional capacity allowed by his pulmonary handicap and overall life situation. Criteria: Non-smokers where activities of daily living are severely limited by dyspnea due to chronic disabling respiratory disease. Medically stable and not requiring more than 4 respiratory interventions per day. Return to home or board and care is a reasonable goal.

Transitional Care

Services represent a distinct part skilled nursing facility within the hospital. It encompasses 63 beds, 31 at Kaweah Delta Rehabilitation Hospital and 32 at the Kaweah Delta Campus. Purpose is to provide quality care to individuals who no longer meet criteria to be in an acute care unit, but continue to require services of a licensed person. The interdisciplinary team works closely together with the physician, patient, and family. We also work with patients who are suffering from a terminal illness. Our focus then changes from one of recovery to one of providing physical, spiritual, and emotional support. For referrals please call (559) 624-3993 and ask for the referral line.
