Medical Records

A patient, or his/her legal representative, may inspect and/or obtain a copy of their medical records, or have copies of medical records sent to another facility. Kaweah Delta Health Care District requires a completed and signed authorization and valid identification before releasing any documents to anyone, including the patient.  

Request a copy of your medical records:

Patients can request copies of their medical records by completing our form entitled “Authorization for Use or Disclosure of Health Information” and by presenting valid identification.  The completed authorization must be dated and signed.  You can download and print the authorization form Download English Authorization From Download Spanish Authorization Form or we can fax or email you an authorization.  Please call 559-624-2218 or click HERE to email us. You may also pick up a form at the address below.

Completed authorization:

The completed authorization may be mailed, faxed or delivered directly to the address below.  

You will now be able to pick up or request a copy of your medical record in the front entrance counter of the Acequia Wing lobby at Kaweah Delta Medical Center.

Processing times may vary depending on the complexity of your request, however in most cases authorizations are completed within two to three business days from the date of receipt.  

Valid identification: 

Medical records will not be released without the requestor providing proper identification. A government issued identification is required. If you are requesting records for someone other than yourself, you must provide supporting legal documentation such as:

  • Power of Attorney
  • Advance Health Care Directive
  • Living Will
  • Executor of the Estate
  • Court order/Conservatorship appointed by the court

Fees for copies of medical records:

If your request is for personal use or to be sent to a third party Kaweah Delta Health Care District may charge a fee providing paper or electronic copies of your medical records. 

Paper: $.25 cents per page.

USB Flash Drive or CD: $5.00

Additional fees for shipping will apply for records to be mailed.

Radiology films:

If you are requesting radiology images, you must contact the Radiology Department at 559-624-2600.  The Health Information Management Department does not process requests for radiology films/CDs.

Patient Portal:

For your convenience you can also view most of your records via our patient portal.  If have already registered for the portal visit  If you are a first time user contact our patient portal line at 559-624-5120.  Patient portal assistance is available Monday - Friday 7 a.m. -3:30 p.m.

How to obtain birth and death certificates: 

Kaweah Delta Medical Center does not retain copies of birth or death certificates. They are maintained by the Tulare County Clerk.
