Your health information is confidential. The Notice of Privacy

Patient Access
Located on the first floor next to the information desk, Patient Access
will take your pre-admission form and provide you with the necessary information
to make your admission into the hospital stress-free. Pre-admission forms are available at the
admissions desk.
Finances
Financial concerns can affect you and your family’s life and ability to
cope with your current illness. Kaweah
Delta has patient financial counselors to assist you with specific questions
regarding financial coverage of your hospital stay. If you believe you might have difficulty
paying your hospital bill, please contact the Financial Counselors at either
(559) 624-2479 or (559) 624-2451.
Patient Accounting
If you have questions regarding your bill once you have been
discharged, please contact Patient Accounting.
Patient Accounting will determine if you are eligible for a fee
reduction or discounted service through our Financial Assistance Program. The hospital will also provide a social
worker that can link you to agencies such as the Employment Development
Department, Social Security office and Tulare County Health and Human Services

Health Information Management
Health Information Management (HIM) processes and manages all medical
records for patients seen at Kaweah Delta.
The HIM department is located in the Support Services Building, 520 W. Mineral King on the 3rd floor and is
open Monday – Friday from 8:00 am - 5:00 pm for your convenience. If you would like information on how to
